Vendor Request: Mail-in Confirmation Sheet

You must be logged in and must fill out the Vendor Request for Participation form before viewing and printing this page.

Your vendor request is NOT YET COMPLETE!

Based on the information you entered, the committee will begin a preliminary review of your application. However, before final review of your application can occur, you must print, sign, and submit this "confirmation sheet" by postal mail. If your application is accepted, you are expected to immediately mail-in a check or money order for the total of your festival fees.
NOTE: Returning vendors in good standing may choose to mail their festival fees with their signed confirmation sheet. If, for any reason, your request for participation is rejected, your check will be returned.
